Biometric and pre-op screenings may be run through your insurance, with the exception of pre-op evaluations for cosmetic surgery. We accept most private and state insurance plans, including Medicare and Medicaid. All other physical fees may be paid directly to our clinic using cash or credit.

Yes, we accept self-pay patients. Payment is due at the time of service.
We will need to know which Medicaid insurance you have (Molina, Iowa Total Care, or Amerigroup), as well as your ID number and your Social Security Number.
