What do I need to bring with me for DOT physical?

If you have chronic medical conditions, a note from your treating provider regarding your treatment and condition is required. Your provider may fill out this form.

Are you in-network with my insurance?

Call the phone number on the back of your insurance card to verify coverage. Our office bills as a primary care clinic. Our office NPI number is 1649394503.

I don’t have insurance. Is that okay?

Yes, we accept self-pay patients. Payment is due at the time of service.

I have Iowa Medicaid, but I do not have a card. Is that okay?

We will need to know which Medicaid insurance you have (Molina, Iowa Total Care, or Amerigroup), as well as your ID number and your Social Security Number.

Do I need an appointment for my drug screen?

You may walk in for your drug screen or book online. Drug screens are performed until two hours before the clinic closes.

Do you offer drive-up testing?

We can perform a virtual visit to assess the patient. After consulting with a physician virtually, we can schedule drive-up testing for minor respiratory infections from your car (influenza, COVID, strep throat).
