Iowa High School Athletic Association (IHSAA) sports physicals: $75.00
All other physicals not billed through insurance: $125.00
DOT physicals not billed through insurance: $175.00

BILLING & INSURANCE
Biometric and pre-op screenings may be run through your insurance, with the exception of pre-op evaluations for cosmetic surgery.
We accept most private and state insurance plans, including Medicare and Medicaid. All other physical fees may be paid directly to our clinic using cash or credit.
